Record-Keeping

The Model Aviculture Program does not prescribe the use of a particular system of record-keeping. Aviculturists should determine which types of records are appropriate to their facilities, types of birds, types of sales and local regulations. In addition to breeding records, such as the samples below, records of veterinary care, shipping documents, permits, contractual agreements, taxes, employee records and other relevant documents should also be maintained for the breeder's protection.

With the exception of the database record which is only a sample, all of the forms below may be printed out for your personal use.
